Overview
This unit integrates taught material with professional and clinical pharmacy practice. In year 4, students:
Demonstrate the ability to identify and resolve pharmaceutical care issues for multiple disease states including comparison of treatment to guidelines, interpretation of patient results, monitoring for efficacy and adverse effects
Use pharmaceutical calculations to recommend drug doses, concentrations and administration rates
Observe and demonstrate interaction and rapport-building with healthcare professionals and patients, focussing on the practice settings of the GP practice and hospital and community pharmacy
Engage in Continuing Professional Development, inter-professional and socially responsible activities to improve employment prospects and develop team-working and communication skills

5-item CPD portfolio (with at least TWO IPE records and a peer discussion) – pass / fail
Medicines reconciliation consultation – pass / fail
Calculations assessment – pass / fail
Attendance – pass / fail (IPE sessions, GP practice placement, community pharmacy placement, 5 clinical placements)

As clinical practice is constantly changing, this material will be reviewed at the start of each semester to ensure it remains relevant.
